Delivery Drivers Part Time Monday-Friday
- good driving record, ability to pass a drug screen and background check
- approximately 25 hours a week
Service Navigator Program Manager-Full Time; Monday-Friday
- Bachelor’s degree in social work, psychology, human services, or related field
- A minimum of 2 years’ experience in human services, geriatrics, or health care setting is desired
ESSENTIAL JOB FUNCTIONS
- Support older adults presenting complex needs
- Develop, implement, and monitor care plans working with the individuals, families, social supports, and health care professionals
- Provide linkage, referral, and advocacy services to various resources
- Administrative duties, including paperwork, documentation, and reporting
- Oversee and maintain the agency’s Caregiver Support Program
- Implement and maintain compliance needs
Volunteer Delivery Drivers are always needed.
Any weekday between the hours of 10am and noon.
